Your first visit to The Foot and Ankle Center establishes a vital foundation for our relationship with you. For your first visit, we'll need to obtain important background information, like your medical history, and allow enough time for you to get to know your doctor. 



Patient Pre-registration
Your completion of the registration forms prior to your visit has a direct impact on our patient flow on the day of your scheduled appointment. If you can return this completed paperwork to us in advance, we will greatly appreciate it! Please use the links below to access and print the forms.

Please Note: If you do not complete and return the patient registration forms prior to your visit, please plan to be at our office at least 15 minutes prior to your appointment time — you will not be considered as checked in until your paperwork is complete. Otherwise, we may have to ask you to reschedule so as not to inconvenience others who do present on time for their appointments.
