Your first visit to The Foot & Ankle Center establishes a vital foundation for our relationship with you. For your first visit, we'll need to obtain important background information, like your medical history, and allow enough time for you to get to know your doctor.



Patient Pre-registration

All new patients, please plan to be at our office at least 15 minutes prior to your appointment time so we can complete your registration — you will not be considered as checked in until your paperwork is complete. Patients who do not arrive 15 minutes prior to their appointment may be asked to reschedule so as not to inconvenience others who do present on time for their appointments.

Please login to the patient portal to complete your registration and enter your medical information. If you need assistance accessing the patient portal, please call our office at 804-320-3668 and the receptionist will help you.
